Youngstown State Athletics Appearance and Outreach Request Form
In accordance with NCAA regulations we, the submitter of this form, do hereby certify that:
- The student-athlete's name, image or appearance is being used in a manner consistent with NCAA Regulations;
- This activity or promotion does not involve co-sponsorship, advertisement or promotion by a commercial agency and the athlete's name, picture or appearance is not used to promote the commercial ventures of any non-profit agency other than through reproduction of the sponsoring company's officially registered regular trademark or logo on printed materials such as pictures, posters or calendars. Personal names, messages, and slogans are prohibited.
- The name or picture of a student-athlete with remaining eligibility may not appear on an institution's printed promotional items (e.g. poster, calendar) that includes a reproduction or a product with which a commercial entity is associated if the commercial entity's officially registered regular trademark or logo also appears on the item;
- The student-athlete cannot miss class.
- All monies derived from the activity or project goes directly to Youngstown State University, Conference office or the charitable, educational, or non-profit agency. The athlete may accept legitimate and normal expenses (i.e. meal) from YSU or the charitable, educational, or non-profit agency related to participation in this activity.
- The student-athlete's name, picture or appearance is not used to promote the commercial ventures of any nonprofit agency;
- The appearance, name or picture of an athlete may be used to promote the fund raising activity of YSU or a charitable, educational or non-profit agency at the location of a commercial establishment. The commercial establishment cannot co-sponsor the event and cannot advertise the presence of the athlete at the commercial location and cannot be involved directly or indirectly in promoting the activity.
- All speaking engagements and appearances must adhere to NCAA regulations and must receive approval by completing the section below for Compliance to review. Reminder: Coaches and Staff are prohibited from participating in and/or attending an event where proceeds benefit prospective student-athletes.
